Everyone attending the club as a player, coach, volunteer or board member must complete a COVID-19 Attestation within 4 hours of planned attendance.
Please bring the email you receive once you have completed the attestation form OR we can scan your phone membership card once you arrive at the club.
YOU WILL NOT BE ALLOWED ONTO CLUB PREMISES WITHOUT A COMPLETED ATTESTATION
